Surgery Eligibility Disclaimer – AZ Pet Spay & Neuter
AZ Pet Spay & Neuter is a limited-service surgery clinic and provides procedures exclusively for healthy pets. To be eligible for surgery through our facility, pets must meet the following criteria:
- We do not accept pets that are:
- Brachycephalic breeds
- Dogs with a distinctive short, broad skull and flattened face, which gives them their unique appearance. Common examples of brachycephalic breeds include Bulldogs, Pugs, French Bulldogs, and Boxers.
- Obese/Emaciated
- Heartworm positive
- Sick or ill
- Senior pets (generally over 7 years old)
- Pets with a history of seizures
- Pets with heart conditions or respiratory issues
- Pets with untreated infections,parasites, or other medical concerns
- Pets currently on steroids or medications that may affect anesthesia
- Any animal deemed high-risk upon pre-surgical evaluation
- Brachycephalic breeds
- Please Note: While not required, we highly recommend pre-anesthetic bloodwork prior to surgery. This helps identify any underlying health issues that may not be visible during a physical exam and allows us to ensure your pet is safe for anesthesia.
We are only able to perform surgeries on pets that are in good overall health. This policy is in place to ensure the safety and well-being of all animals in our care, as our facility is not equipped to manage high-risk procedures or emergency interventions.

How Our Surgery Center Appointments Work
To ensure a smooth and safe surgery day for you and your pet, please review the steps below carefully.
After You Book Your Appointment
1. Appointment Confirmation Email
Once your appointment is successfully scheduled, you will receive a confirmation email from JotForm that includes your appointment date.
IMPORTANT:
If your confirmation email does not include an appointment date, the form was not completed correctly or fully, and your appointment is not scheduled. Please complete the form again or contact our team for assistance.
The Day Before Surgery
2. Consent Forms & Instructions
The day before surgery, all required consent forms and pre-op information will be sent to the appointment maker via text and email. Please review and complete all forms promptly to avoid delays.

3. Arrival & Check-In
On the day of surgery, all pet parents will arrive at their selected drop-off time to:
